Axe Valley Dementia Team and Willow Groups
The Axe Valley Dementia Team is based in Axminster Hospital and offers specialist dementia assessment and support.
We provide this service for people who live in Axminster and Seaton areas. Covering the Axminster Medical Practice and Seaton and Colyton and Townsend House Medical Practice patients.
The team provide an up to 6 week assessment of someone’s health and social needs in their own home. The assessment time also allows the person to get to know some of the team and gain some idea of how we can help and support through the social aspect of our service in Willow day groups. The team also provide ongoing assessment and support throughout the different stages of dementia.
Activities
Social isolation is a very real risk as we get older and those with dementia can become quickly isolated and this can make their condition worse. We therefore provide group activities that focus on social interaction, cognitive stimulation and above all enjoyment.
They include
- Art and craft- including making our own greetings cards and planning decorations for our tree in the St Mary’s Christmas Tree festival
- Music, singing and dancing
- Quizzes, reminiscence conversations and table top games
- Keeping engaged in the community
We always enjoy a chance for a tea party and often celebrate birthdays and other key events throughout the year.
As well as the support we can offer the person who attends with us we can also offer guidance and support for their carers, acting as a link between other services and offering practical advice.
